NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

Under the Illinois Health Facilities Planning Act, notice is given of the receipt of an application for an exemption of a change in ownership of O'Fallon Surgical Center, LLC, an ambulatory surgical treatment center located at 741 Insight Avenue, O'Fallon, Illinois (E-018-26).  Currently, Dr. Shakeel Ahmed ("Seller") owns 88.44% of the operating entity, O'Fallon Surgical Center, LLC, and 100% of Haris Assets, LLC, which owns the facility's real estate. Dr. Michael Stock of Ideal Eye Surgery, LLC ("Buyer") holds the remaining 11.56% operating interest. Under the terms of the asset purchase agreement, Dr. Stock will acquire all of Dr. Ahmed's operating interest, yielding 100% ownership of O'Fallon Surgical Center, LLC through Insight Surgical Holdings, LLC, and acquire the real estate through a separate entity, Insight741 LLC, for a total fair market value of $9,250,000.  

Post-closing, O'Fallon Surgical Center, LLC will remain the licensed operating entity at the same location, but its clinical focus will transition into a single-specialty center dedicated exclusively to ophthalmic surgical services integrated with Ideal Eye Surgery, LLC's clinical management infrastructure.

This exemption application was called substantially complete on August 31, 2026.  Written requests for a public hearing (no emails or faxes will be accepted) must be received by the Illinois Health Facilities and Services Review Board by September 14, 2026.    The Application can be viewed at the offices of the Illinois Health Facilities and Services Review Board at the address below, or at the State Board’s website https://hfsrb.illinois.gov/
