NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt to establish an Open-Heart Surgery category of service at Northwestern Medicine Lake Forest Hospital. Project #25-033, Northwestern Medicine Lake Forest Hospital, Lake Forest. Applicant: Northwestern Lake Forest Hospital and Northwestern Memorial Healthcare. The Applicants propose to establish the Open-Heart Surgery category of service in the existing surgical space at its hospital located at 1000 North Westmoreland Road, Lake Forest. Project cost: $2,840,000.

The application contained a Safety Net Impact Statement and was deemed substantially complete on August 20, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. HFSRB staff must receive requests for hearings by September 4, 2025. The public may also submit written comments on this project. All public written comments must be received by December 26, 2025. HFSRB will post its findings in a HFSRB Staff Report, which will be available via the Internet on September 11, 2025. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
