NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt of an Application for Permit to discontinue Elmhurst Outpatient Surgery Center, LLC, 1200 S. York Road, Suite 1400, Elmhurst, Illinois, Project #25-038. Applicants: Elmhurst Outpatient Surgery Center, LLC, Endeavor Health, and Elmhurst Memorial Hospital. There is no cost to this project.  

The application was deemed substantially complete on October 20, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. To obtain a copy of an application, please call the office for details and copying fees at the number listed below. Consideration by HFSRB has been tentatively scheduled for the January 13, 2026, meeting. Any person wanting a public hearing on the proposed project must submit a written request for a hearing to:

HFSRB staff must receive requests for a hearing by November 4, 2025. The public may also submit written comments on this project. All public written comments must be received by December 24, 2025. HFSRB will post its findings in an HFSRB Staff Report, which will be available online on December 31, 2025. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
