NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

Under the Illinois Health Facilities Planning Act, notice is given of the receipt of an application for an exemption of a change in ownership.  Fresenius Medical Care Chicagoland, LLC (an existing joint venture between Bio-Medical Applications of Illinois, Inc. [51%] and AIN Ventures, LLC [49%]) plans to acquire Fresenius Medical Care Highland Park, a 20-station End-Stage Renal Disease (ESRD) facility located at 1657 Old Skokie Road, Highland Park, IL. (E-016-26) The facility is currently owned and operated by Fresenius Medical Care of Illinois LLC. Post-transaction, Fresenius Medical Care Chicagoland, LLC will become the certified entity and Medicare holder. In connection with the acquisition, Fresenius Medical Care Chicagoland, LLC will pay $3,152,683 to Fresenius Medical Care Illinois, LLC for the Fresenius Medical Care Highland Park facility. The expected completion date is November 1, 2026.

This proposal is considered a change of ownership among related persons resulting in a license being issued to an entity different from the current licensee.   This exemption application was deemed substantially complete on August 3, 2026. Written requests for a public hearing (no emails or faxes will be accepted) must be received by the Illinois Health Facilities and Services Review Board by August 18, 2026.
