26-028 Ignite Nursing and Rehabilitation

NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish a Long-Term Care (LTC) facility in New Lenox. Project #26-028, Ignite Nursing & Rehabilitation – New Lenox. Applicants: Ignite Medical New Lenox, LLC, New Lenox Senior Partners, LLC, New Lenox Investors, LLC, New Lenox Manager, LLC, LBG New Lenox, LLC, Ignite New Lenox JV, LLC. The applicants propose to establish a 96-bed LTC facility in 62,590 GSF of newly constructed space located at 1900 Silver Cross Boulevard, New Lenox. Project cost: $30,451,641.

The application contained a Safety Net Impact Statement and was declared complete on June 15, 2026. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office, at the address below. Consideration by the State Board has been tentatively scheduled for the November 5, 2026, State Board Meeting. Requests for hearings must be received by this Agency no later than June 30, 2026. Any person wanting to submit written comments on this project must submit these comments by October 16, 2026.  The State Board will post its findings in a State Board Staff Report, which will be made available on the internet on October 22, 2026. The public may submit written responses in support of or in opposition to the findings of the Illinois Health Facilities and Services Review Board.
