26-015 Fresenius Medical Ross Dialysis - Englewood

NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to discontinue an End Stage Renal Dialysis (ESRD) facility in Chicago.  Project #26-015, Fresenius Medical Care Ross Dialysis-Englewood, Chicago.  Applicants: Ross Dialysis-Englewood, and Fresenius Medical Care Holdings, Inc.  The applicants propose to discontinue their 24-station ESRD facility, located at 946 West 63rd Street, Chicago.  There are no project costs.   The application contained a Safety Net Impact Statement and was declared complete on April 2, 2026.  A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office, at the address below.  To obtain a copy of an application, please call the office at the number listed below for details and copying fees.  Consideration by the State Board has been tentatively scheduled for the September 15, 2026, State Board Meeting.  Any person wanting a public hearing on the proposed project must submit a written request for a hearing to:

Requests for hearings must be received by this Agency no later than April  17, 2026.  Any person wanting to submit written comments on this project must submit these comments by August 26, 2026.   The State Board will post its findings in a State Board Staff Report, which will be made available on the internet on August 31, 2026.  The public may submit written responses in support of or in opposition to the findings of the Illinois Health Facilities and Services Review Board.  The public will have until 9:00 am, September 8, 2026.  The internet address used to access this report is: www.hfsrb.illinois.gov
