NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

By the Illinois Health Facilities Planning Act requirements, notice is given of the receipt of an application for exemption for a change of ownership of Cardiovascular Institute Ambulatory Surgery Center, 10 Marin Avenue, Naperville, Illinois (#E-005-25).  This change of ownership is considered a stock transfer, resulting in no change in the licensee.  The fair market value of the transaction is $402,000. 

The Applicants are Cardiovascular Institute Ambulatory Surgery Center, LLC., Edward Hospital, Endeavor Health, and MCI ASC, LLC. MCI ASC, LLC will acquire a 60% ownership interest in the ASTC from Edward Hospital, while Edward Hospital will maintain a 40% interest. The licensee will remain Cardiovascular Institute Ambulatory Surgery Center, LLC, and the site owner is Ryan Companies US, Inc. 

This exemption application was called substantially complete on January 21, 2026.  Written requests (no emails or faxes will be accepted) for a public hearing must be received by the Illinois Health Facilities and Services Review Board no later than February 5, 2026.    The Application can be viewed at the offices of the Illinois Health Facilities and Services Review Board at the address below or at the State Board’s website https://hfsrb.illinois.gov/
