NOTICE OF PUBLIC HEARING AND WRITTEN COMMENT

Following the Illinois Health Facilities Planning Act requirements, notice is hereby given of the receipt of an application for exemption for a change of ownership of Freeport Memorial Hospital, 1045 W. Stephenson Street, Freeport, Illinois (#E-018-25).  The Applicants are Freeport Regional Health Care Foundation, Freeport Memorial Hospital, and Mercy Health Care Corporation. The transaction is considered a change in the sponsorship or control of the person who is licensed, certified, or owns the physical plant and assets of a health care facility. The Freeport Regional Health Care Foundation and Mercy Health Corporation propose to amend their respective governing documents to name MercyHealth as the sole corporate member of the Freeport Regional Health Care Foundation.

This project is scheduled for the November 18, 2025, State Board Meeting.  The public may also submit written comments on this project. All public written comments must be received by October 29, 2025. HFSRB will post its findings in an HFSRB Staff Report, which will be available online on November 5, 2025. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
