NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Pursuant to the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is hereby given of receipt of an Application for Permit to establish an Ambulatory Surgical Treatment Center performing vascular and podiatry procedures, at Midwest Surgical Center, 13610 Route 59, Plainfield, Illinois. Project #25-037 Applicant: Midwest Surgical Center, LLC The project cost is $4,009,035.

The application was deemed substantially complete on October 23, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. Consideration by HFSRB has been tentatively scheduled for the February 26, 2026, meeting. Any person wanting a public hearing on the proposed project must submit a written request for a hearing to:

HFSRB staff must receive requests for a hearing by November 7, 2025. The public may also submit written comments on this project. All public written comments must be received by February 7, 2026.  HFSRB will post its findings in an HFSRB Staff Report, which will be available online on February 13, 2025. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
