On August 15, 2025, the Governor signed Senate Bill 798. This bill amends Section 3 of the Health Facilities Planning Act by revising the definition of "Non-clinical service area" and by establishing new definitions for "Patient care unit" and "Provider". 25-006 Maple Avenue Kidney Center Establishment

NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish an End Stage Renal Dialysis (ESRD) facility in Melrose Park.  Project #25-006, Maple Avenue Kidney Center, Melrose Park.  Applicants:  Oak Park Kidney Center, LLC, d/b/a Maple Avenue Kidney Center, LLC.  The applicant proposes establishing an 18-station ESRD facility located at 1111 Superior Avenue, Suite 204, Melrose Park, as a replacement for the 18-station ESRD facility recently discontinued via Application #25-004 in Oak Park.  Project cost: $665,992.

The application contained a Safety Net Impact Statement and was declared complete on February 11, 2025.  A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office, at the address below.  To obtain a copy of an application, please call the office for details and copying fees, at the number listed below.  Consideration by the State Board has been tentatively scheduled for the August 12, 2025, State Board Meeting.  Any person wanting a public hearing on the proposed project must submit a written request for a hearing to:

Requests for hearings must be received by this Agency no later than February 26, 2025.  Any person wanting to submit written comments on this project must submit these comments by July 23, 2025.  The State Board will post its findings in a State Board Staff Report, and the report will be made available via the Internet on July 29, 2025.  The public may submit written responses in support of or in opposition to the findings of the Illinois Health Facilities and Services Review Board.  The public will have until 9:00 am, August 4, 2025.  The internet address used to access this report is:
