NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

Following the Illinois Health Facilities Planning Act requirements, notice is hereby given of the receipt of an application for exemption for a change of ownership of THE REAL ESTATE ONLY housing Dialysis Centers of America-Illinois, Inc. d/b/a Fresenius Kidney Care Elmhurst, 133 E Brush Hill Road, Suite 410, Elmhurst, Illinois (#E-019-25).  This transaction is considered the sale or transfer of the physical plant and related assets of a healthcare facility, which does not result in a change of ownership or control of the current licensee. The fair market value of the transaction is $4,998,950. The Applicants are Welltower OM Group LLC, Welltower, Inc., Elmhurst 133 MP WRK7, LLC, KAREP VII Reit, LLC. The owner of the site is Elmhurst 133 MP WRK7, LLC.  

This exemption application was called substantially complete on October 17, 2025.   Written requests (no emails or faxes will be accepted) for a public hearing must be received by the Illinois Health Facilities and Services Review Board by November 1, 2025.    The Application can be viewed at the offices of the Illinois Health Facilities and Services Review Board at the address below, or the State Board’s website https://hfsrb.illinois.gov/
