NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

In accordance with the Illinois Health Facilities Planning Act, notice is given of the receipt of an application for exemption to discontinue the open-heart category of service at Endeavor Health Northwest Community Hospital, 800 West Central Road, Arlington Heights, Illinois (E-006-26). The Applicants are Endeavor Health Northwest Community Hospital and Endeavor Health.  The expected completion date is June 27, 2026.  

This exemption application was deemed substantially complete on May 15, 2026. Written requests (no emails or faxes will be accepted) for a public hearing must be received by the Illinois Health Facilities and Services Review Board no later than May 30, 2026.
