NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt to establish an Ambulatory Surgery Treatment Center (ASTC) in Shiloh.  Project #26-001, Eye Surgery Specialists LLC.  Applicant: Eye Surgery Specialists LLC.  The Applicant proposes to establish a single-specialty ASTC in 7,504 GSF of space located at 847 North Green Mount Road, Shiloh.  Project cost: $5,048,737.

The application contained a Safety Net Impact Statement and was deemed substantially complete on January 20, 2026. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. HFSRB staff must receive requests for hearings by February 5, 2026. The public may also submit written comments on this project. All public written comments must be received by May 13, 2026.  HFSRB will post its findings in an HFSRB Staff Report, which will be available via the Internet on May 19, 2026. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
