NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt to discontinue an Ambulatory Surgery Treatment Center (ASTC), in Calumet City.  Project #25-046, Midwest Eye Center S.C.  Applicants:  Opening Day Acquisition 111, LLC, and Midwest Eye Center S.C.  The Applicants propose to discontinue services at its single specialty ASTC, located at 1700 East West Road, Calumet City.  Project cost: $0.00.

The application contained a Safety Net Impact Statement and was deemed substantially complete on December 3, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. To obtain a copy of an application, please call the office for details and copying fees at the number listed below. Consideration by HFSRB has been tentatively scheduled for the June 3, 2026, meeting. Any person wanting a public hearing on the proposed project must submit a written request for a hearing to:

HFSRB staff must receive requests for hearings by December 18, 2025. The public may also submit written comments on this project. All public written comments must be received by May 13, 2026.  HFSRB will post its findings in an HFSRB Staff Report, which will be available via the Internet on May 19, 2026. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
