NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

By the Illinois Health Facilities Planning Act requirements, notice is given of the receipt of an application for exemption for a change of ownership of Soderstrom Dermatology Center, S.C., dba Peoria Ambulatory Surgery Center, 4909 N Glen Place, Peoria, Illinois. This change of ownership is considered a stock transfer resulting in no change in the licensee(#E-004-25). The licensee is Soderstrom Dermatology Center, S.C., dba Peoria Ambulatory Surgery Center, and the site owner is Store Master Funding XII, LLC.

This exemption application was called substantially complete on June 23, 2025. Written requests (no emails or faxes will be accepted) for a public hearing must be received by the Illinois Health Facilities and Services Review Board no later than July 8, 2025. The Application can be viewed at the offices of the Illinois Health Facilities and Services Review Board at the address below or at the State Board’s website https://hfsrb.illinois.gov/
