E-009-26 Carle Health Proctor Hospital - Discontinuation of AMI Service

NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

Under the Illinois Health Facilities Planning Act, notice is given of the receipt of an application for an exemption for the discontinuation of the 18-bed acute mental illness category of service at Carle Health Proctor Hospital, 5409 Knoxville Ave., Peoria, Illinois (E-009-26).  The Applicants are The Carle Foundation d/b/a Carle Health and Proctor Hospital d/b/a Carle Health Proctor Hospital.  There is no cost to the project, and the expected completion date is 120 days after State Board approval.  

This exemption application was deemed substantially complete on May 19, 2026. Written requests for a public hearing (no emails or faxes will be accepted) must be received by the Illinois Health Facilities and Services Review Board by June 3, 2026.
