NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt to establish a Medical Office Building (MOB) in Chicago. Project #25-009, TARGET Health Cooperative Medical Clinics Building. Applicant: The Board of Trustees of The University of Illinois. The Applicant proposes establishing a Medical Office Building in 29,700 GSF of space, located at 5525 South Pulaski Road, Chicago. Project cost: $34,190,818.

The application was deemed substantially complete on February 14, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. To obtain a copy of an application, please call the office for details and copying fees at the number listed below. Consideration by HFSRB has been tentatively scheduled for the May 6, 2025, meeting. Any person wanting a public hearing on the proposed project must submit a written request for a hearing to:

HFSRB staff must receive requests for hearings by March 3, 2025. The public may also submit written comments on this project. All public written comments must be received by April 14, 2025. HFSRB will post its findings in a HFSRB Staff Report, which will be available via the Internet on April 21, 2025. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
