NOTICE OF A PUBLIC HEARING AND OPPORTUNITY FOR PUBLIC COMMENT

Per the Illinois Health Facilities Planning Act requirements, notice is given of the receipt of an application for a permit for the discontinuation of Ascension Saint Elizabeth-Chicago, located at 1431 North Claremont, Chicago, Illinois (Project #24-030). A Public Hearing has been requested and will be conducted by the Staff of the Illinois Health Facilities and Services Review Board on Wednesday, December 11, 2024, at the

The State Board has tentatively scheduled a meeting for March 18, 2025, to consider this Project. Anyone wanting to submit written comments on this project must do so by February 26, 2025. Comments can be sent to DPH.HFSRB@Illinois.gov. The State Board will post its findings in a State Board Staff Report online on March 4, 2025. The public may submit written responses in support of or in opposition to the findings of the Illinois Health Facilities and Services Review Board. The public will have until 9:00 am, March 10, 2025.
