NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt to discontinue an Ambulatory Surgery Treatment Center (ASTC), in Chicago. Project #25-048, NovaMed Surgery Center of Chicago-Northshore, Chicago. Applicants: NovaMed Surgery Center of Chicago-Northshore, LLC, Surgery Partners, Inc., Lawrence D. Wolin M.D., Gerstein ASC Interests, LLC, David M. Greenberg, M.D. The Applicants propose to discontinue services at its single specialty ASTC, located at 3034 West Peterson Avenue, Chicago. Project cost: $0.00.

The application contained a Safety Net Impact Statement and was deemed substantially complete on December 10, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. HFSRB staff must receive requests for hearings by December 26, 2025. The public may also submit written comments on this project. All public written comments must be received by May 13, 2026. HFSRB will post its findings in a HFSRB Staff Report, which will be available via the Internet on May 19, 2026. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
