IMPORTANT REMINDERS

  • All correspondence must be received through HFSRB’s official email address (dph.hfsrb@illinois.gov).  Correspondence submitted through other emails will not be considered received and will not be processed or reviewed.
  • Applications for Certificate of Need permits, Certificate of Exemptions, and requests for Extension of Financial Commitment, Permit Renewal, and Permit Alteration can be submitted via dph.hfsrb@illinois.gov.  In lieu of this process, applications and requests can be submitted to HFSRB’s principal office:  525 West Jefferson Street, 2nd Floor, Springfield, Illinois 62761.  At this time, all fees associated with the above-referenced applications and requests must be mailed to HFSRB's principal office in Springfield.
  • HFSRB will take no action on any request that requires a fee until the fee payment is received at HFSRB’s principal office.
  • Effective July 1, 2025, revisions to HFSRB’s rules at 77 Ill. Adm. Code 1130 take effect.  These changes center on definitions, technical assistance, fees, extension of financial commitment, permit renewal, and permit alterations.  Applications for permit or exemption, and post-permit transactions received by HFSRB on or after July 1, 2025, will be subject to the new rules and fee requirements.

25-026 OSF Heart of Mary Medical Center

NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT


Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt to reduce beds and discontinue categories of service at an acute care hospital in Urbana. Project #25-026, OSF Heart of Mary Medical Center, Urbana. Applicants: OSF Healthcare System, and OSF Healthcare System d/b/a Heart of Mary Medical Center. The Applicants proposes to reduce its Medical/Surgical bed capacity from 110 to 25 beds, and discontinue its Pediatrics, Intensive Care (ICU), Comprehensive Physical Rehabilitation, Open Heart Surgery and Cardiac Catheterization categories of service, reducing the overall bed count from 156 to 40 beds. The hospital is located at 1400 West Park Street, Urbana. Project cost: $0.00.


The application contained a Safety Net Impact Statement and was deemed substantially complete on July 1, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. Consideration by HFSRB has been tentatively scheduled for the November 18, 2025, meeting. HFSRB staff must receive requests for hearings by July 16, 2025. The public may also submit written comments on this project. All public written comments must be received by October 29, 2025. HFSRB will post its findings in a HFSRB Staff Report, which will be available via the Internet on November 5, 2025. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff. All written public comments about the HFSRB staff findings must be received before 9:00 am, November 10, 2025. The internet address used to access this report is:
