NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to discontinue an End Stage Renal Dialysis (ESRD) facility in Deerfield. Project #24-040, Fresenius Medical Care Deerfield, Deerfield. Applicants: Fresenius Medical Care North America and Fresenius Medical Care of Illinois, LLC d/b/a Fresenius Medical Care of Deerfield. The applicants propose discontinuing its 12-station ESRD facility at 405 Lake Cook Road, Deerfield. There are no project costs.

The application contained a Safety Net Impact Statement and was declared complete on January 2, 2025. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office at the address below. Consideration by the State Board has been tentatively scheduled for the June 24, 2025, State Board Meeting. Any person wanting a public hearing on the proposed project must submit a written request for a hearing to:

Requests for hearings must be received by this Agency no later than January 17, 2025. Any person wanting to submit written comments on this project must submit these comments by June 4, 2025.

The State Board will post its findings in a State Board Staff Report online on June 10, 2025. The public may submit written responses in support of or in opposition to the findings of the Illinois Health Facilities and Services Review Board. The public will have until 9:00 am, June 16, 2025.
