E-008-26 Carle Health Proctor Hospital - Change of Ownership

NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

Under the Illinois Health Facilities Planning Act, notice is given of the receipt of an application for an exemption to change the ownership of Carle Health Proctor Hospital, 5401 N. Knoxville, Illinois (E-008-26). This change of ownership, among related persons, results in a license being issued to an entity different from the current licensee. The fair market value of the transaction is $34,583,508. The estimated completion date is November 1, 2026.

The Carle Foundation d/b/a Carle Health seeks to consolidate Proctor Hospital d/b/a Carle Health Proctor Hospital under the legal entity and hospital license of The Methodist Medical Center of Illinois d/b/a Carle Health Methodist Hospital. The Applicants are The Carle Foundation d/b/a Carle Health, The Methodist Medical Center of Illinois d/b/a Carle Health, Methodist Hospital, and Proctor Hospital d/b/a Carle Health Proctor Hospital.

This exemption application was deemed substantially complete on May 19, 2026. Written requests for a public hearing (no emails or faxes will be accepted) must be received by the Illinois Health Facilities and Services Review Board by June 3, 2026.
