NOTICE OF AN OPPORTUNITY FOR PUBLIC COMMENT AND REQUEST FOR PUBLIC HEARING

Under the Illinois Health Facilities Planning Act, notice is given of the receipt of an application for an exemption of a change in ownership. Fresenius Medical Care Chicagoland, LLC (a joint venture between Bio-Medical Applications of Illinois, Inc. [51%] and AIN Ventures, LLC [49%]) plans to acquire Fresenius Medical Care Mundelein (Exemption Project # E-017-26).  The facility is a 14-station End-Stage Renal Disease (ESRD) facility located at 1402 Townline Road, Mundelein, IL. It is currently owned and operated by Fresenius Medical Care of Illinois LLC. Post-transaction, Fresenius Medical Care Chicagoland, LLC will become the certified entity and Medicare certification holder.  The Fair Market Value is $2,745,276. The expected completion date is November 1, 2026.  

This proposal is considered a change of ownership among related persons that results in the issuance of a license to an entity other than the current licensee. This exemption application was deemed substantially complete on August 3, 2026. Written requests for a public hearing (no emails or faxes will be accepted) must be received by the Illinois Health Facilities and Services Review Board by August 18, 2026.
