17-002 Mercy Health Hospital and Medical Center Crystal Lake-Hospital, Crystal Lake

​NOTICE OF PUBLIC HEARING  AND OPPORTUNITY FOR WRITTEN COMMENT 

In accordance with the requirements of the Illinois Health Facilities Planning Act (20 ILCS 3960) Notice is given of a Public Hearing on an application for permit for the establishment of an acute care hospital, in Crystal Lake.  Project #17-002 - Mercy Health Hospital and Medical Center Crystal Lake-Hospital, Crystal Lake.  The applicants are Mercy Crystal Lake Hospital and Medical Center, Inc., and Mercy Health Corporation.  The applicants propose to establish an acute care hospital, containing eleven (11) medical/surgical (Med/Surg) beds, and two (2) Intensive Care (ICU) beds.  The 13-bed facility will be established in proximity to the applicants proposal to establish a medical office building (MOB) (Project #17-001), located on the southeast corner of State Route 31 and Three Oaks Road, in Crystal Lake.  The project cost is $81,710,315. 

The Public Hearing is to be held by the Staff of the Illinois Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act.  The Hearing is open to the public and will afford an opportunity for parties with interest to present written and/or verbal comment relevant to the project.  All allegations or assertions should be relevant to the need for the proposed project and be supported with two copies of documentation or materials that are printed or typed on paper, size 8 1/2" by 11". 

The Public Hearing will be held on Tuesday, February 21, 2017 from 1:30 P.M. – 3:30 P.M. with public sign-in at 1:15 P.M. It will be conducted at the City Hall of Crystal Lake, Council Chambers located at 100 West Woodstock Street, Crystal Lake, Illinois.

The application was declared complete on January 26, 2017.  A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office, at the address below.  Consideration by the State Board has been tentatively scheduled for the June 20,  2017 State Board Meeting.  Any person wanting to submit written comments on this project must submit these comments by May 31, 2017.
