17-057 Valley Ambulatory Surgery Center, St. Charles

LEGAL NOTICE OF PUBLIC HEARING AND OPPORTUNITY FOR WRITTEN COMMENT

In accordance with the requirements of the Illinois Health Facilities Planning Act and 77 Illinois Adm. Code Part 1130, Notice is given of a Public Hearing on an application for permit for a proposed construction and/or modification project (Project #17-057).  This project proposes to discontinue and establish an ambulatory surgical treatment center (ASTC) in St. Charles, Illinois.  The applicants are Valley Ambulatory Surgery Center, L.P., VASC, Inc., and Surgery Partners, Inc.  The applicants propose to discontinue an existing ASTC located at 2210 Dean Street, Saint Charles, Illinois and establish an ASTC located at 2475 Dean Street, Saint Charles, Illinois.  The cost of the project is $16,618,319.

The Public Hearing is to be held by the Illinois Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act.  The Hearing is open to the public and will afford an opportunity for parties at interest to present written and/or verbal comment relevant to the project.  All allegations or assertions should be relevant to the need for the proposed project and be supported with two copies of documentation or materials that are preferably printed or typed on paper size 8 ½" by 11". 

The Public Hearing will be held on Thursday, January 18, 2018 from 11:15 a.m. – 1:00 p.m. at the City of St. Charles Municipal Building, Council Chamber, 2 East Main Street, St. Charles, Illinois.  Public sign-in for this hearing will begin at 11:00 a.m.

The application was declared complete on October 23, 2017.  Consideration by the State Board has been tentatively scheduled for the February 27, 2018 State Board Meeting.  For additional information contact Courtney Avery, Administrator at 312.814.4825 or courtney.avery@illinois.gov.
