NOTICE OF REVIEW AND OPPORTUNITY FOR PUBLIC HEARING AND WRITTEN COMMENT

Per the requirements of the Illinois Health Facilities Planning Act [20 ILCS 3960/], notice is given of receipt to relicense/reestablish an Ambulatory Surgery Treatment Center (ASTC) in Peoria.  Project #26-004, The Center for Health Ambulatory Surgery Center-II, Peoria.  Applicants: The Center for Health Ambulatory Surgery Center and OSF Healthcare System.  The Applicants propose to re-establish a multi-specialty ASTC, located at 303 North William Kumpf Boulevard, Peoria.  Project cost: $0.00.

The application contained a Safety Net Impact Statement and was deemed substantially complete on January 30, 2026. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board (HFSRB) Office at the address below. To obtain a copy of an application, please call the office for details and copy fees at the number listed below. Consideration by HFSRB has been tentatively scheduled for the July 21, 2026, meeting. HFSRB staff must receive requests for hearings by February 16, 2026. The public may also submit written comments on this project. All public written comments must be received by July 1, 2026.  HFSRB will post its findings in an HFSRB Staff Report, which will be available via the Internet on July 7, 2026. The public may submit written responses in support of or in opposition to the findings of the HFSRB staff.
