NOTICE OF PUBLIC HEARING AND OPPORTUNITY FOR WRITTEN COMMENT
In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish an Ambulatory Surgery Treatment Center (ASTC), in Elgin. Project #16-038, Advocate Sherman Ambulatory Surgery Center, Elgin. Applicants: Advocate-SCA Partners, LLC, SCA-Illinois, LLC, SCA Surgery Holdings, LLC, Surgical Care Affiliates, LLC, Surgical Care Affiliates, Inc., Evangelical Services Corporation, Advocate Health Care Network, Advocate Sherman Hospital, Advocate Sherman Ambulatory Surgery Center, LLC. The applicants propose to establish a multi-specialty ASTC in 15,010 GSF of leased space located at 1445 North Randall Road, Elgin. Project cost: $12,718,852.
The Public Hearing is to be held by the Staff of the Illinois Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act. The Hearing is open to the public and will afford an opportunity for parties with interest to present written and/or verbal comment relevant to the project. All allegations or assertions should be relevant to the need for the proposed project and be supported with two copies of documentation or materials that are printed or typed on paper, size 8 1/2" by 11".
The Hearing will be held on Friday, November 4, 2016 from 11:00 A.M. – 1:00 P.M. with sign-in at 10:30 A.M. It will be conducted at the Gail Borden Public Library located at 270 N Grove Avenue, Elgin IL 60120.
The application contained a Safety Net Impact Statement and was declared complete on September 12, 2016. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office, at the address below. Consideration by the State Board has been tentatively scheduled for the January 24, 2017 State Board Meeting. Any person wanting to submit written comments on this project must submit these comments by January 4, 2017.
