IMPORTANT REMINDERS

  • All correspondence must be received through HFSRB’s official email address (dph.hfsrb@illinois.gov).  Correspondence submitted through other emails will not be considered received and will not be processed or reviewed.
  • Applications for Certificate of Need permits, Certificate of Exemptions, and requests for Extension of Financial Commitment, Permit Renewal, and Permit Alteration can be submitted via dph.hfsrb@illinois.gov.  In lieu of this process, applications and requests can be submitted to HFSRB’s principal office:  525 West Jefferson Street, 2nd Floor, Springfield, Illinois 62761.  At this time, all fees associated with the above-referenced applications and requests must be mailed to HFSRB's principal office in Springfield.
  • HFSRB will take no action on any request that requires a fee until the fee payment is received at HFSRB’s principal office.
  • Effective July 1, 2025, revisions to HFSRB’s rules at 77 Ill. Adm. Code 1130 take effect.  These changes center on definitions, technical assistance, fees, extension of financial commitment, permit renewal, and permit alterations.  Applications for permit or exemption, and post-permit transactions received by HFSRB on or after July 1, 2025, will be subject to the new rules and fee requirements.
  • In addition to our changes, the Illinois General assembly updated its webpage today.  As a result, all our links to our administrative rules need to be updated.  On the Board’s website, links to all our rules are here:  Administrative Rules

LEGAL NOTICE OF PUBLIC HEARING AND OPPORTUNITY FOR WRITTEN COMMENT

In accordance with the requirements of the Illinois Health Facilities Planning Act and 77 Illinois Adm. Code Part 1130, Notice is given of a Public Hearing on Project 20-011, the modernization and the expansion of the medical surgical and intensive care beds at Northwestern Memorial Hospital, 251 East Huron Street, Chicago. The project cost is $77,607,985.

The Public Hearing is being conducted by the Illinois Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act. The Hearing is open to the public with opportunity to present relevant verbal and written comments on the proposed project.

The Public Hearing will be held on Monday, June 15, 2020 beginning at 9:30AM and concluding at 10:30AM at George W. Dunne Cook County Office Building, 69 West Washington Street, 22nd Floor, Chicago, Illinois.

No later than Wednesday, June 10th at 5PM, information detailing how to access the public hearing virtually and/or via conference will be posted on the HFSRB website at www2.illinois.gov/sites/hfsrb. Any questions, please leave a voice mail message at 312-590-6511. In accordance with public health guidelines anyone entering and remaining in the George W. Dunne Cook County Office Building will be required to wear a face covering at all times and adhere to social distancing standards.
