NOTICE OF PUBLIC HEARING AND OPPORTUNITY FOR WRITTEN COMMENT
In accordance with the requirements of the Illinois Health Facilities Planning Act, notice is given of receipt to establish a Health and Fitness Center, in DeKalb. Project #16-054, Northwestern Medicine Kishwaukee Health and Fitness Center, DeKalb. Applicants: Northwestern Memorial Healthcare, KishHealth System, and Kishwaukee Community Hospital. The applicants propose to construct a 2-story health and fitness center in 111,105 GSF of space, located on the campus of Northwestern Medicine Kishwaukee Hospital, 626 East Bethany Road, DeKalb. Project cost: $46,392,770.
The Public Hearing is to be held by the Staff of the Illinois Health Facilities and Services Review Board pursuant to the Illinois Health Facilities Planning Act. The Hearing is open to the public and will afford an opportunity for parties with interest to present written and/or verbal comment relevant to the project. All allegations or assertions should be relevant to the need for the proposed project and be supported with two copies of documentation or materials that are printed or typed on paper, size 8 1/2" by 11".
The Public Hearing will be held on Thursday, February 9, 2017 from 11:30 A.M. – 1:00 P.M. with public sign-in at 11:15 A.M. It will be conducted at the DeKalb Public Library, Bilder Family Meeting Room located at 309 Oak Street, DeKalb, Illinois 60115-3369.
The application declared complete on December 29, 2016. A copy of the application may be viewed at the Illinois Health Facilities and Services Review Board Office, at the address below. Consideration by the State Board has been tentatively scheduled for the March 14, 2017 State Board Meeting. Any person wanting to submit written comments on this project must submit these comments by February 28, 2017.
