E-026-16 Castle Surgicenter, Aurora

In accordance with the requirements of the Illinois Health Facilities Planning Act, Notice is given of a receipt of a change of ownership of a health care facility application #E-026-16 –Castle Surgicenter, LLC.  Castle Surgicenter, LLC is a multi-specialty ambulatory surgical treatment center located at 2111 Ogden Avenue, Aurora, Illinois.  The applicants are Rush-Copley Surgicenter LLC, 2111 Ogden Avenue, Aurora, Illinois, Copley Memorial Hospital, Inc. and Rush-Copley Medical Center, Inc., 2000 Ogden Avenue, Aurora, Illinois.  The operating entity licensee will be Rush Copley Surgicenter, LLC and the owner of the site is TPSS, LLC, 2111 Ogden Avenue, Aurora, Illinois.  The fair market value of the facility is $14,303,190.  The date of the proposed transaction is October 1, 2016. 

Rush-Copley Surgicenter, LLC, proposes to purchase the assets of Castle Surgicenter, LLC.  The new licensee will be Rush-Copley Surgicenter, LLC, d/b/a Rush-Copley Surgicenter.  Rush-Copley Surgicenter, LLC, is a newly created limited liability corporation that is a subsidiary of Copley Memorial Hospital, Inc.  Copley Memorial Hospital is the sole member of Rush-Copley Surgicenter, LLC.  Copley Memorial Hospital currently owns and operates two licensed healthcare facilities; Copley Memorial Hospital in Aurora and Copley Memorial Hospital Freestanding Emergency Center in Yorkville.  Rush-Copley Medical Center, Inc., is the parent of Copley Memorial Hospital.  After this transaction is completed, Rush-Copley Surgicenter will continue to operate as a multi-specialty ambulatory surgical treatment center in leased space in its current location.  Rush-Copley Surgicenter will continue to lease the site and building from TPSS, LLC, the current owner of the site and building in which the ambulatory surgical treatment center is located.  The transaction is considered a purchase resulting in the issuance of a license to an entity different than the current licensee.

This exemption application for the change of ownership was declared complete on July 27, 2016.  Written requests for a public hearing must be received by the Illinois Health Facilities and Services Review Board no later than August 11, 2016.  Request for a public hearing must be sent by letter to the administrator at the address below.  No emails or faxes will be accepted
