E-018-16 - Justice Med-Surg Center dba Forest Med-Surg Center, Justice
In accordance with the requirements of the Illinois Health Facilities Planning Act, Notice is given of a receipt of a change of ownership of a health care facility application #E-018-16 – Justice Med-Surg. Center, Ltd. d/b/a Forest Med-Surg. Center. The applicants are the Surgery Center of Illinois, LLC, 8701 W. 95th Street, Oak Lawn, Illinois, and Justice Med-Surg. Center, Ltd., 9050 W. 81st Street, Justice, Illinois. The operating entity/licensee is Justice Med-Surg. Center, Ltd a multi-specialty ambulatory surgical treatment center. The owner of the real estate is First Step Holdings, LLC.
The change of ownership transaction proposes the Surgery Center of Illinois, LLC to acquire seventy-five percent (75%) ownership interest in Justice Med-Surg. Center, Ltd. The eight current inpidual owners of Justice Med-Surg. Center Ltd. propose to reduce their current ownership shares and collectively retain twenty-five percent (25%) ownership interest in Justice Med-Surg. Center Ltd.
The applicants do not propose a change in the scope of services or levels of care, as the applicants will continue to provide services under the approved ASTC service lines, including: Dermatology; Gastroenterology; General Surgery; Obstetrics/Gynecology; Ophthalmology; Orthopedics; Plastic Surgery; Podiatry; and Urology.
The Surgery Center of Illinois, LLC will use unrestricted cash from internally available financial resources to purchase the ownership interest for an aggregate price of $1,275,000. Fair Market Value of the facility was calculated at $1,700,000 and was computed using a multiple of earnings methodology.
This exemption application for the discontinuation was declared complete on July 11, 2016. Written requests for a public hearing must be received by the Illinois Health Facilities and Services Review Board no later than July 26, 2016. Request for a public hearing must be sent by letter to the administrator at the address below. No emails or faxes will be accepted.
