E-017-16 - Tri-Cities Surgery Center, LLC, Geneva
In accordance with the requirements of the Illinois Health Facilities Planning Act, Notice is given of a receipt of the discontinuation of a health care facility application #E-017-16 – Tri-Cities Surgery Center, LLC. The applicants are Northwestern Memorial HealthCare, 251 East Huron Street, Chicago, CDH-Delnor Health System d/b/a Cadence Health, 25 North Winfield Road, Winfield, Delnor-Community Hospital, 300 Randall Road, Geneva, and Tri-Cities Surgery Center, LLC, 345 Delnor Drive, Geneva.
The applicants are proposing the discontinuation of Tri-Cities Surgery Center located at 345 Delnor Drive, Geneva, a multi specialty ambulatory surgical treatment center. On March 10, 2016, the Chair of the Health Facilities and Services Review Board approved a Change of Ownership exemption request #E-013-16 which allowed Delnor Community Hospital to acquire 100% of the membership interest in Tri-Cities Surgery Center, LLC. This transaction was completed on June 1, 2016.
The applicants are seeking approval for the discontinuation of the Ambulatory Surgery Treatment Center license so as to operate the facility as an extension of Delnor-Community Hospital Peri-operative Services department. No services will be discontinued as a result of this request; all peri-operative activities will now be operated under the hospital license
All medical records will be located at Delnor Community Hospital and will be maintained in accordance will all federal and state law. There will be no impact on access to care as residents of the community will be provided the same care and service at the same location. A Safety Net Impact Statement was provided as required.

This exemption application for the discontinuation was declared complete on July 8, 2016. Written requests for a public hearing must be received by the Illinois Health Facilities and Services Review Board no later than July 25, 2016. Request for a public hearing must be sent by letter to the administrator at the address below. No emails or faxes will be accepted.
